i've been dabbing around with 3d level design for maybe four or five years now, so i have a bit of experience with projects (in my case, single maps) that take thousands of hours to complete. an example of my notes:

...
RAKNIR/OLD CAVE
--CART RIDE
----MAKE THE AMBIENTSOUND MOVER (X)
----BUILD THE RAIL SUPPORTS
----BUILD THE POWERING MECHANISM ON BOTH SIDES
----CONSIDER MAKING CART A TOGGLE MOVER? (X)
--CART CAVE
----ADD TARYDIUM CRYSTALS
----ADD DREDGING PIPES
----BUILD LIGHTING FIXTURES
----VERTEX EDIT LAST CAVE BRUSH (X)
--OLD MINE ENTRANCE
----BUILD DOCK
----BUILD MAIN STRUCTURE
----BUILD GIANT DOORS AND ASSOC. MOVERS (X)
...

the (X) marks mean i've completed that bit so far. obviously, there is no order to which i get things done (whatever i'm inspired to do at a given time, i'll do). every couple of weeks i remove the completed stuff from the list and add a couple more things to do. it isn't step-by-step, but a to-do list, which is good enough.
